Liberty Hospital’s $60 million development announced by its board of trustees will be located near the hospital’s campus and will provide services to patients who have left the hospital and to those who need rehabilitation. The Health Living Community development will incorporate inpatient and outpatient services with a focus on prevention and management of chronic conditions, rather than on costly and reactive acute care, Liberty Hospital CEO David Feess said.

Liberty Hospital will develop the facility in partnership with Action Pact, a senior care “culture change” company based in Manhattan, Kan., and Healthy Living Centers of America, a West Des Moines, Iowa, firm specializing in wellness and integrated care.

Feess said the hospital is providing property for the project and, depending on the financing structure, might invest financially. Action Pact has expertise in post-acute care for seniors, he said.
